1. Carries a caseload of post adoption clients.
2. Assists with information and referral for
TDFPS contracts, including services to families out of state that are eligible
for post adoption services through the contract.
3. Responsible for documenting and determining
client eligibility, maintaining case records, assessing third party resources
and ensuring these resources are applied when available, assessing client
needs, developing a sound written service plan and updating the service plan in
compliance with TDFPS contract standards.
4. Assesses adopted child’s need for placement
outside the home and applies for level-of-care determination if the child
requires such care.
5. Works to find community resources that
support families that have adopted TDFPS children, with emphasis on finding
Spanish speaking providers. These might
include, but are not limited to, therapists, psychologists, psychiatrists,
respite providers, etc.
6. Supporting family functioning by attending
meetings with them in the community, such as ARD meetings, CRCG’s, or MHMRA
meetings.
7. Reviews billing from subcontractor to the post
adoption program for direct client caseload, approves billing or addresses
billing issues with subcontractor.
8. Participates in case staffings, supervisory
conferences, and staff continuing education.
9. Performs other tasks as assigned by supervisor.
Family Coordinator:
1. Carries a caseload of families in recruitment
stages of orientation, application processing and training.
2. Creates and monitors the recruited family
files meet TDFPS contract standards.
3. Establishes outreach sites at community
neighborhoods representative of the population served.
4. Develops as well as represents Spaulding for
Children at special events, expos, churches, etc.

and follow-up on background checks and references on foster and adoptive
families
7.
Develop
public service announcements for radio, television, newspapers, etc.
8.
Create
media/community relationships and opportunities for enhancing awareness
9.
Maintain
public display materials, i.e. brochures, display boards, etc.
10.
Participate
in community collaborations, initiatives, etc.
11.
Performs
other tasks as assigned by supervisor
REQUIRED
EDUCATION AND EXPERIENCE
Bachelor’s Degree in Social Work, or related
area, with one year of child welfare or child placing experience.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
Good family and individual assessment skills. Ability to work independently or as part of a
team. Possess strong writing, speaking,
and organizational skills with ability to cope with changing situations and
work under pressure.
