POST ADOPTION CASE MANAGER

Spanish Speaking Case Manager

 

Position:  Part-time, salary, exempt
Location: Houston
Supervisor:  Reports to Supervisor, Post Adoption Program


GENERAL DESCRIPTION

The person in this position is a member of the post adoption program team and will work with the Supervisor of the post adoption program to provide information and referral services to all Spanish speaking post adoption families and other post adoption families seeking services, as needed.  The Caseworker will work to maintain an effective working relationship between all Spaulding for Children programs in order to provide a continuum of service to adopted children and their families with the main emphasis being on casework services for Spanish speaking families.  Casework consists of the planning and coordination of post adoption services to an adopted child and their family in order to help the family and child adjust to the adoption, cope with any history of abuse in the child’s background, and avoid permanent or long-term removal of adopted children from the family. Bi-lingual in Spanish preferred. 

RESPONSIBILITIES

  1. Carries a caseload of post adoption clients, including all Spanish speaking families.
  2. Assists with information and referral for TDFPS contract, including services to families out of state that are eligible for post adoption services through the contract.
  3. Responsible for documenting and determining client eligibility, maintaining case records, assessing third party resources and ensuring these resources are applied when available, assessing client needs, developing a sound written service plan and updating the service plan in compliance with TDFPS contract standards..
  4. Assesses adopted child’s need for placement outside the home and applies for level-of-care determination if the child requires such care.
  5. Works to find community resources that support families that have adopted TDFPS children, with emphasis on finding Spanish speaking providers.  These might include, but are not limited to, therapists, psychologists, psychiatrists, respite providers, etc.
  6. Supporting family functioning by attending meetings with them in the community, such as ARD meetings, CRCG’s, or MHMRA meetings. 

  7. Reviews billing from subcontractor to the post adoption program for direct client caseload.

REQUIRED EDUCATION AND EXPERIENCE

Bachelor’s Degree in Social Work, or related area, with one year of child welfare or child placing experience.  Bi-lingual in Spanish/English. 

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Good family and individual assessment skills.

Valid Texas Drivers License

Valid auto liability insurance

Willingness to work evenings and weekends as required

POST ADOPTION CASE MANAGER

Spanish Speaking Case Manager / Family Coordinator 

Position:  Full-time, salary, exempt
Location: Corpus Christi
Supervisor:  Reports to Supervisor, Post Adoption and Supervisor, Recruitment/Training

GENERAL DESCRIPTION

The person in this position is a member of both the post adoption program team and the recruitment team.  The case manager will maintain an effective working relationship between all Spaulding for Children programs in order to provide a continuum of service to adopted children and their families.

This case manager will work 50% time with the Supervisor of the post adoption program to provide information and referral services to post adoption families and other post adoption families seeking services, as needed.    Case management consists of the planning and coordination of post adoption services to an adopted child and their family in order to help the family and child adjust to the adoption, cope with any history of abuse in the child’s background, and avoid permanent or long-term removal of adopted children from the family.

This case manager will also dedicate 50% time to recruitment as a Family Coordinator.  This position actively seeks to identify and train families for children with special needs. 

RESPONSIBILITIES:

Post Adoption Case Manager

1.      Carries a caseload of post adoption clients.

3.      Responsible for documenting and determining client eligibility, maintaining case records, assessing third party resources and ensuring these resources are applied when available, assessing client needs, developing a sound written service plan and updating the service plan in compliance with TDFPS contract standards.

4.      Assesses adopted child’s need for placement outside the home and applies for level-of-care determination if the child requires such care.

5.      Works to find community resources that support families that have adopted TDFPS children, with emphasis on finding Spanish speaking providers.  These might include, but are not limited to, therapists, psychologists, psychiatrists, respite providers, etc.

6.      Supporting family functioning by attending meetings with them in the community, such as ARD meetings, CRCG’s, or MHMRA meetings.

7.      Reviews billing from subcontractor to the post adoption program for direct client caseload, approves billing or addresses billing issues with subcontractor.

8.      Participates in case staffings, supervisory conferences, and staff continuing education.

9.      Performs other tasks as assigned by supervisor.

 

Family Coordinator:

1.      Carries a caseload of families in recruitment stages of orientation, application processing and training.

2.      Creates and monitors the recruited family files meet TDFPS contract standards.

3.      Establishes outreach sites at community neighborhoods representative of the population served.

4.      Develops as well as represents Spaulding for Children at special events, expos, churches, etc.

5.      Input initial family information in database and put together family records

6.      Process and follow-up on background checks and references on foster and adoptive families

7.      Develop public service announcements for radio, television, newspapers, etc.

8.      Create media/community relationships and opportunities for enhancing awareness

9.      Maintain public display materials, i.e. brochures, display boards, etc.

10.  Participate in community collaborations, initiatives, etc.

11.  Performs other tasks as assigned by supervisor

REQUIRED EDUCATION AND EXPERIENCE

Bachelor’s Degree in Social Work, or related area, with one year of child welfare or child placing experience.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Good family and individual assessment skills.  Ability to work independently or as part of a team.  Possess strong writing, speaking, and organizational skills with ability to cope with changing situations and work under pressure.
